ADMISSIONS PROCESS

1.Contact Focus12 on 01284 701702 or by email on info@focus12.co.uk

2. Assessment date will be arranged and basic details taken.

3. Staff member will contact you within 3 days to go through detailed assessment form.

4.Visit the centre for half an hour to meet staff and get an impression of Focus12.

5.Attend Access to treatment Group each Wednesday until place becomes available

6. Admission

This process would be standard for clients needing to arrange funding from their local Health Authority. Clients who were funding their treatment some other way could possibly by-pass much of the information gathering steps.

Whenever possible we would encourage new clients to come and have a look at Focus12 and get a feel of what the centre is like. It is going to be your 'home' for between 8 - 10 weeks and it is important you feel comfortable.
